Volunteer Forms

Every volunteer at HCA is required to complete a set of volunteer forms that are available by contacting the main office.

If you volunteer at HCA you are required to have a current police check/criminal record check (CRC) on file with the school.

Police checks/CRCs need to be renewed every two years.
Forms are automatically sent to parents of enrolled students at the start of each school year.
